United Way of North Idaho is not currently accepting applications for the Community Care Fund Grant. UWNI has an updated grant process which will be awarded as a one-time payment. The next round of CCF grants will be in June and applications are due by April 15, 2019. Nonprofit organizations who have received funding in the past will notice a slight change in the reporting structure. As in recent years, our focus is to make an impact in the areas of education, financial stability, and health. With that in mind, our grant application and reporting fall within those impact areas, with education split into School Readiness and Youth Success. Feel free to take a look around the application, and when you're ready to submit, follow the instructions below.

Step 1: Request username and password by emailing [email protected]. Requests are typically handled within a day or two, but please allow one week to process. You may complete the application without a login but will be unable to save your work

Step 2: Determine which impact area most closely aligns with the program for which you are seeking funding. Program examples can be viewed by clicking the icons of each impact area. Program examples are provided to help agencies identify which impact area a program will fit; these are common examples of programs but are by no means comprehensive. If you are unsure about which impact area to select for your program, please contact us for guidance. 

 

Education: School Readiness

Helping youth and children achieve their potential. Children (birth through age 5) enter school ready to learn and succeed.

Education: Youth Success

Children do well in school, graduate on time, and are ready to succeed in post-secondary education and a career path.

Financial Stability

Individuals and families attain financial stability by lowering costs and accessing benefits; individuals attain family-sustaining jobs that offer the opportunity for advancement; individuals have adequate transportation.

Health & Wellness

Individuals and families lead healthy lives through access to quality medical services including physical, dental, and behavioral health; prevent and respond to family violence, child abuse and neglect, and suicide; encompasses efforts to create healthy systems and environments.

 

Step 3: Carefully review the reporting requirements in the Results Framework.

In addition to program-specific metrics, all funded agencies must be able to report:
  1. Number of unduplicated individuals served by this grant (specifying population served--students/children/adults/seniors, etc)
  2. County or counties served
  3. Quantity of volunteers and volunteer hours engaged
  4. Number of low-moderate income and ALICE households participating in or benefitting from the program (LMI= at or below 80% Area Median Income) 

Step 4: Complete the application below. The deadline for the fall cycle is Monday, October 15th, 2018 by 5:00pm. Incomplete or late applications will not be reviewed by the committee.

Please note, agencies selected to receive funding will be required to provide a certificate of liability and worker's compensation insurance as well as a completed IRS W-9 Form prior to receiving funds.

Please provide a brief summary of your program that is suitable for a public-facing document (75-100 words)
Please provide a narrative of your program and include the following information: a. How the program is addressing the identified UWNI impact area; b. The goals of the program, creating a bullet point for each goal; c. The evidence-based practice(s) used to achieve those goals; d. The population served through this initiative and in what communities they reside; e. Activities to be covered by this funding
Please demonstrate how the organization is positioned to implement the program, including: a. Key personnel (board, staff, volunteers); b. Leveraging diverse stakeholders and resources; c. Collaborations and partnerships
Provide a timeline of the program that outlines each goal, benchmarks, and outcomes.
Review the quantity measures in the Results Framework and select one that best describes the client services and activities provided by the program. Estimate the number of clients to be served by the program during the grant period. Enter this number as the program's target quantity measure, e.g. CS-Q3 target: 200 children K-3 receiving literacy supports
Review the results indicators in the same impact area as the quantity measures selected above. Select one or more indicators that the program will track and report during the grant period. Enter the selected results indicators as the program's target results indicators. E.g. CS-R3 target: 60% of children K-3 served will improve literacy skills
